Establishing California Employer Pull Notice (EPN) Account
This article explains the process for meeting California’s requirements for monitoring Drivers through the California Employer Pull Notice (EPN) Program as part of J. J. Keller’s MVR Monitoring Service. Employers must obtain Driver consent, ensure Drivers operate on behalf of the business, and comply with DMV information use restrictions. The process involves 3 main steps: (1) creating a California EPN account, (2) applying for an EPN Requestor Code (R-Code), and (3) establishing SambaSafety as the authorized agent by submitting the required authorization and consent forms. Employers with an existing EPN account can skip directly to Step 3. Processing times vary, with the EPN Requestor Code typically taking 3–5 business days and final state approval 2–3 weeks.
In order to monitor drivers licensed in the state of California, California requires the use of the California Employer Pull Notice (EPN) monitoring program.
California defines who is required to have a California EPN account and criteria of who needs to be enrolled, however, California Vehicle Code (CVC) §1808 allows for employers to enroll drivers who do not meet this criteria assuming the following conditions are met:
- The driver consents to being enrolled in EPN (complete an Authorization for Release of Driver Record Information (INF 1101) (PDF) and maintain the form at your principal place of business).
- The driver is an employee of the EPN account holder. If the driver is not an employee (for example, volunteer), you can still enroll them if they operate a vehicle on behalf of your business.
- DMV information may not be shared and must be for the exclusive business use of the employer.
- Upon termination of employment, the driver must be immediately deleted from the EPN account.

There are 3 main steps to establish the California EPN account for use of MVR Monitoring:

Step 1: Create your California EPN Account
- Navigate to the CA EPN Customer Portal
- Select Create an Account
- Follow the onscreen steps to enter your email, and select continue
- Complete the CAPTCHA authentication
- Locate the confirmation email sent to the address you entered
- Open the email and either select Confirm your email link (or copy the URL link and paste it into a new browser window)
- Enter the Name and a Phone Number you have immediate access to
- Choose to Receive Code via Text or Receive Code via Phone Call and select Continue
- Enter the code provided into the CA EPN Customer Portal and select the Confirm Phone Number
- Create a password following the listed criteria and select Continue
- Please note there is an option to show your password
- The account has now been created.
- Select Click here to log in
- You will receive a confirmation email
- Log in using the credentials you created from the login screen.
- Select Text Me or Call Me to receive the verification code and select Continue
- Enter the code provided and select Confirm Phone Number
Step 2: Apply for EPN Requestor Code
To request a new Employer Pull Notice (EPN) R-Code (also known as Requestor Code), choose Apply for a New Employer Pull Notice (EPN) Requester Code. It directs you to the INF 1104 application.
- Complete INF 1104 application
- Add one existing driver
- If you have more than 1 California driver, they will be added once SambaSafety has been approved as your Authorized Agent
- The CA EPN Requestor Code approval can take 3-5 business days
- The Create an Online Payment Account link directs you to the portal validation steps and is not a required step for a SambaSafety Customer to complete
step 3: Establish SambaSafety as your Authorized Agent
Once a California EPN account has been set up and an assigned R-Code (also known as Requestor Code), the person who established the EPN account would need to complete the Agent Authorization Packet comprised of the following forms:
- California INF 2110 – EPN Agent Authorization
- California INF 1128 – Annual Re-Certification
- SambaSafety End User Consent to Provide Access to California Electronic Pull Notice Data
California INF 2110 – EPN Agent Authorization:
- Section 1 must be filled out based on the information on the California EPN account which can be found on the approval letter received from the state or a recent EPN notice
- Section 2 is already pre-filled with the required information. No need to fill in the Requester Code in Section 2.
- Complete the section on page 2
California INF 1128 - Annual Re-Certification
- Page 1 must be completed by the person who established the EPN account -- please keep this for your records and resign on an annual basis on page 2
SambaSafety End User Consent to Provice Access to California Electronic Pull Notice Data:
- Name of End User should be your company name
- Service Provider should be listed as SambaSafety
This paperwork along with the approval letter from the state or a recent EPN notice would need to be supplied back to your J.J. Keller contact for submittal.
It can take 2-3 weeks for the state of California to process the paperwork
