Upload MVR Release Form

This article explains how to upload a signed Acknowledgment & Authorization/Driver Release Form in Encompass for MVR Monitoring enrollment. After the Driver signs the form, navigate to Drivers > Driver Qualification, select the Driver, and locate the MVR Release item in the Maintenance Checklist. Use the paperclip icon to upload or drag the signed form into the system. Once uploaded, a pop-up will prompt you to certify specific requirements before the Driver is officially enrolled.

Once the Acknowledgement & Authorization/Driver Release Form has been signed by your driver/s, it needs to be uploaded and attached to the driver's MVR Request checklist item in Encompass. Go to Drivers > Driver Qualification and then select the driver that needs the form uploaded.

Scroll to the Maintenance Checklist selection. Hover over the MVR Release line and click the attachment/paper clip icon.

Upload/drag the signed form to the upload tool. Close when finished.

A pop-up will display requiring you to certify various items in order to officially enroll the driver after uploading the Release Form.
